Membership FAQs
Is TDW Membership an insurance plan?
Our membership plan is not insurance and is not a qualified health plan under the Affordable Care Act.
Are memberships refundable?
You can receive a full refund for up to 30 days after purchase if no benefits of the plan have been used.
Do memberships expire?
Membership is valid for 365 days from date of purchase. Any unused benefits are non- refundable and do not roll over to the next year.
Can I combine membership with dental insurance?
The membership plan benefits may not be combined with insurance, other offers, or discounts.
Which procedures qualify for the 20% discount?
Treatment completed at Thrive Dental Wellness qualifies for the 20% discount. Our membership plan does not cover third party lab diagnostic testing or procedures completed by specialists outside of our practice. Payment for treatment rendered at the office is due at time of service.

As a courtesy to all of our insurance based patients, we are pleased to help you complete and submit forms to enable you to maximize use of your dental benefits. You must supply all necessary insurance information prior to your visit.

- We do not “treat your insurance plan.” We will treat you and address your individual needs. You have the right to accept or decline any treatment recommended.
- Your insurance benefits are determined by your specific dental plan which is negotiated between your employer and the insurance company. Our office accepts some PPO plans. We do not accept Medicaid at this time.
- Our office is not, and cannot be a part of the legal contract between you and your insurance company.
- Keep in mind that dental benefit plans will never pay for completion of your dental care. It is only meant to assist you and reduce your out of pocket costs.
